Taking into account Professor James Reason states errors are seen as consequences rather than causes, having their origins not so much in the perversity of human nature as in “upstream” systemic factors, and that these include recurrent error traps in the workplace and the organizational processes that give rise to them, a documented tool or procedure must be designed and executed so organizations have operational capability to monitor latent conditions at work in terms of identifying gaps affecting operational personnel’s human performance.
